Our Collective Purpose:

The Whitehall Community Health Action Team (CHAT) will examine and improve the health of people in our community.


Our Targeted Area of Impact

Increase the number of people in Whitehall with a usual and ongoing source of primary healthcare, starting with a focus on women.

The Whitehall CHAT is a diverse group of individuals who represent Whitehall city government, the school system, local businesses, local churches, healthcare representatives, and the county health department. This group is called a Community Health Action Team because there is an emphasis on action and making an impact in the community. The group started meeting back in the Spring of 2014, and it selected access to care as one of Whitehall’s biggest challenges.

What is Primary Care?

John's Hopkins University describes primary care as "the level of a health services system that provides entry into the system for all new needs and problems, provides person-focused (not disease-oriented) care over time, provides care for all but very uncommon or unusual conditions, and coordinates or integrates care, regardless of where the care is delivered and who provides it. It is the means by which the two main goals of a health services system, optimization and equity of health status, are approached."


Why is it important?

It is important to establish a relationship with a primary care provider and see him or her regularly. Primary care providers can treat temporary illnesses, but they can also prevent or catch health issues before they become more serious. In addition, they can refer you to a specialist or other healthcare provider if needed. Urgent care centers are also important health systems because you can be seen the same day without an appointment, and the cost is usually much less than an emergency room visit.

The "Community Health Resources" tab provides contact information for providers and healthcare professionals in the Whitehall area. There is information for pediatricians, primary care providers, mental health providers, and urgent care centers.
